Measure
One visit sets your baseline — labs and your full history.
If you’ve done everything right and the weight still comes back, the problem isn’t willpower — it’s biology. Custom GLP-1 protocols, comprehensive testing, and a physician who actually answers.

“Losing it fast isn’t the goal. We titrate toward about 1% of body weight per week — enough to keep the progress steady, slow enough to protect muscle and avoid the side effects that make people quit. Across the practice, the average works out to roughly 0.6% a week.”
All members treated for weight loss since May 2025 with a follow-up weight on file. Median 4.8 months of care. Members on maintenance plans excluded. Individual results vary.
How it works
Your numbers decide the plan. One physician runs it — until you don’t need him.
01
One visit sets your baseline — labs and your full history.
02
Dr. Saremi reads every number himself and writes your protocol.

Every week — until you hit your goal, then you off-ramp.
At goal · the exit
Your exit is planned from day one — taper, monitoring, maintenance. Most programs are designed to keep you. This one isn’t.
No care team, no handoffs. You work directly with Dr. Saremi — board-certified in Clinical Informatics, turning medical data into decisions — and he has run this loop hundreds of times.

The honest answer depends on where you’re starting from. If you’re starting with obesity or severe obesity, I won’t promise you a finish line — in the manufacturers’ own trials, people who stopped abruptly regained most of what they lost within a year, and I’d be lying if I told you otherwise. If you’re closer to the middle — twenty or thirty pounds — there can be a pathway off, and I’ve had members make it: we stretch the dose out gradually, then stop, and keep watching. Either way, you don’t stay at full intensity forever — when you hit your goal, you step down to a Maintenance plan. What I promise is the truth about which case you’re in, starting at the consult.
Membership is $249, $499, or $999 a month depending on tier — that includes your visits, labs, care plan, and direct access to me. GLP-1 medication is not included and is billed separately; the price depends on which medication and dose you need, so we’ll walk through your exact all-in number on the consult before you commit to anything.
I’m licensed in California, Texas, Florida, Illinois, Arizona, and Georgia. All visits are telemedicine, with in-person options in the San Diego area.
You get it from a licensed pharmacy, not from me — I write the prescription and it’s filled and billed there. I prescribe brand-name, FDA-approved GLP-1s only. I don’t prescribe compounded versions: with a brand-name pen you know exactly what’s in it and exactly what dose you took, and that’s the whole basis for titrating against your labs.
Labs are included in your membership — you’ll never see a separate lab bill from us. We don’t route them through insurance, which is what lets us order the panels I actually want rather than what a payer approves.
That depends on your plan. Essential members message me on weekdays, 10am to 5pm PT. Premium and Longevity members reach me seven days a week, after hours included — that is part of what those tiers buy. Either way you get a same-day reply, and either way you are messaging me, not a portal and not a care coordinator.
Your PCP is important, and I work alongside them, not instead of them. The difference is time and focus: a typical primary care visit is about 15 minutes covering everything; our visits are 30 to 60 minutes about your metabolic health specifically — dosing, labs, body composition, and what’s actually changing. And between visits, you message me directly, not a portal.
