GLP-1 weight-loss care by video in Texas
Yes, a doctor can prescribe Wegovy, Zepbound and other GLP-1 medications to you by video in Texas, as long as the doctor holds a Texas medical license. Dr. Adonis Saremi does. He's an obesity medicine physician based in La Jolla, California, and he sees Texas patients by video on the Essential plan, $249 a month. Medication is billed separately by the pharmacy.

Texas lets a doctor treat you by telemedicine only with a full Texas medical license. The separate out-of-state telemedicine license Texas used to issue is closed to new doctors. 22 TAC 175.1
For a new patient, Texas sets a floor for the first visit: a live video visit, or your records reviewed together with a live phone call. Dr. Saremi's first visit is a 30-minute live video visit. If you agree, Texas law also has the doctor send a report of the visit to your primary care doctor within 72 hours. Texas Occupations Code 111.005
GLP-1 medications are not controlled substances, so no extra federal registration is involved. Your Texas pharmacy fills the prescription like any other.
How it works
- A free 15-minute consult. You ask your questions and hear what your own monthly total would be before you commit to anything.
- A 30-minute first visit by video. Labs and your full history set your baseline. Dr. Saremi reads every number himself and writes your plan.
- A 15-minute check-in by video every month. Your dose is adjusted from what your numbers show.
- Messaging on weekdays, with a same-day reply. In Texas that's noon to 7 pm Central time (11 am to 6 pm in El Paso).
You need to be in Texas, or another state where he's licensed, at the time of each visit. He holds medical licenses in California, Arizona, Georgia, Illinois and Texas, and a Florida telehealth registration.
What it costs
Membership: $249 a month on the Essential plan, or $224 a month if you pay for the year. It covers your visits with Dr. Saremi, your care plan and messaging him directly. HSA and FSA cards are accepted.
Medication is not included. Your prescription is filled and billed by the pharmacy. Dr. Saremi prescribes brand-name, FDA-approved GLP-1s only, never compounded versions.
If you pay the manufacturer's self-pay price, these are the published monthly prices as of October 2026:
- Wegovy pen: $349 ($199 for a new patient's first two fills at the two starting doses, through December 31, 2026). NovoCare
- Zepbound: $299 at 2.5 mg, $399 at 5 mg, and $449 at 7.5 to 15 mg if you refill within 45 days. Lilly
- Wegovy pill: $149 to $299, depending on the dose. NovoCare
So a month on the Essential plan with a Wegovy pen at the self-pay price comes to $598. With Zepbound it runs from $548 to $698, depending on the dose. Labs and other tests are billed separately, only when you need them.
Who you'll see
Every visit is with Dr. Adonis Saremi himself. He is board-certified in Internal Medicine, Obesity Medicine and Clinical Informatics, trained in internal medicine at UC San Diego, and was the Chief Medical Officer of Noom. Between visits you message him directly, not a portal or a care coordinator.
His offices are in La Jolla and Newport Beach, California. Patients outside California see him by video.
Questions people in Texas ask
- Can a California doctor prescribe a GLP-1 to me in Texas?
- Only if that doctor also holds a Texas medical license, because Texas law follows where the patient is. Dr. Saremi holds one, issued by the Texas Medical Board, and you can check it on the Board's website.
- Do I ever need to travel to California?
- No. Texas patients see Dr. Saremi by video. His offices are in La Jolla and Newport Beach if you ever want to visit, but nothing in your care requires it.
- Does the practice bill my insurance?
- No. Membership is paid directly, and HSA and FSA cards are accepted. The medication is billed by the pharmacy, and the manufacturers' self-pay prices are listed above.
- What happens if I'm traveling during a visit?
- Each visit has to happen while you're in a state where Dr. Saremi can treat you: California, Arizona, Florida, Georgia, Illinois or Texas. If you'll be somewhere else, move the visit.
