Wegovy Pill vs. Injection (and Foundayo): 3 Differences That Matter

By Adonis Saremi, MD

The Wegovy pill and the Wegovy shot are the same drug, semaglutide. In separate trials the shot averaged 14.9% weight loss and the pill 13.6%. The pill needs a strict morning routine, and paying cash it costs less: $149 to $299 a month, against $349 for the shot. Foundayo, the other weight-loss pill, is a different drug from Lilly that you can take with or without food. Here are the three differences that matter when you choose.

1. How you take it

Wegovy pill (oral semaglutide). Take it first thing in the morning on an empty stomach, with no more than 4 ounces of water. Then wait at least 30 minutes before you eat, drink or take any other pills. If you take thyroid medicine, tell whoever prescribes it: in the label’s interaction study, the pill raised levothyroxine levels in the blood by about a third.

Foundayo (orforglipron). A new molecule from Lilly. You can take it with or without food. If you use the birth control pill, add a backup method for 30 days after you start and after every dose increase.

2. How much weight comes off

These figures come from four separate trials. None of these medicines has been tested head to head against another, so treat the comparison as a rough guide. Each number is the average loss counting everyone who started treatment, whether or not they kept taking it.

MedicineAverage weight lossTrial length
Wegovy pill (25 mg)13.6% (placebo 2.4%)64 weeks
Foundayo, top dose (17.2 mg)11.1% (placebo 2.1%)72 weeks
Wegovy shot (2.4 mg)14.9% (placebo 2.4%)68 weeks
Zepbound shot, top dose (15 mg)20.9%72 weeks

3. What you’ll pay in cash

Self-pay prices from the makers’ own programs, checked October 7, 2026. They change, so confirm before you fill.

MedicineSelf-pay price a month
Wegovy pill$149 (1.5 mg), $199 (4 mg), $299 (9 mg and 25 mg)
Foundayo$149 (0.8 mg and 2.5 mg), $199 (5.5 mg), $299 (9 mg; 14.5 mg and 17.2 mg if you refill within 45 days). The 2.5 mg and 5.5 mg prices run through December 31, 2026
Wegovy shot$349 (0.25 mg to 2.4 mg). New patients pay $199 for the first two fills through December 31, 2026
Zepbound shot$299 (2.5 mg), $399 (5 mg), $449 (7.5 mg to 15 mg) if you refill on time

Which one fits you

If needles are what’s stopping you, and you can keep the morning routine, a pill is a real option now. If you want the most weight loss, the shots still lead.
