Physician-led weight loss and metabolic care in La Jolla
By Adonis Saremi, MDUpdated
Dr. Adonis Saremi, an internist board-certified in Obesity Medicine, runs a physician-led weight loss and metabolic practice at 888 Prospect St in La Jolla, with video visits in six states.
What does weight loss care with Dr. Saremi include?
- A 30-minute first visit, where he goes through your history, your medicines and your goals, and orders your labs.
- A monthly check-in, and follow-up visits as often as you need them.
- Direct messaging with Dr. Saremi himself.
- Labs, included in both plans.
- A prescription when one fits you, with brand-name GLP-1s only, never compounded.
- On Premium, four 60-minute in-person visits a year at the La Jolla or Newport Beach office.
Is this concierge medicine?
Yes, for weight and metabolic health. Dr. Saremi runs a concierge practice: a monthly, self-pay membership with one physician, where you see Dr. Saremi himself at every visit and message him between visits.
It isn’t concierge primary care. You keep your own doctor for physicals, sick visits and everything outside metabolic health, and he works alongside them.
What does it cost?
Membership pays for Dr. Saremi’s time: visits with him and messaging between visits.

Which weight loss medicines does Dr. Saremi prescribe?
Brand-name GLP-1 medicines, chosen with you at your first visit. In their main trials, people on the top dose of Zepbound lost about 21% of their body weight over 72 weeks, and people on the Wegovy shot about 15% over 68 weeks, on average. These were separate trials, never tested head to head, and a trial average isn’t a promise for any one person.
- Zepbound (tirzepatide): approved for weight management, and for moderate to severe obstructive sleep apnea in adults with obesity.
- Wegovy (semaglutide): approved for weight management, as a weekly shot or a daily pill.
- Ozempic (semaglutide) and Mounjaro (tirzepatide): the same two medicines, approved for type 2 diabetes.
Dr. Saremi sets your dose to what your body needs, not a fixed schedule. For patients who do well on a lower dose, that can cut medication costs by up to half.
Is it only about weight?
No. Dr. Saremi is an internist as well as an obesity medicine physician, and he treats the conditions that travel with weight: blood sugar and prediabetes, fatty liver, sleep apnea, heart risk, and longevity care.
How do I reach Dr. Saremi, and who adjusts my dose?
Message Dr. Saremi directly and hear back the same day, Monday to Friday, 10 to 5. Premium members can message 7 days a week, after hours included. Same-day video visits are often available on both plans. In-person visits at La Jolla or Newport Beach (Premium) are booked at least a day ahead.
The person who reads your labs is the person who writes your prescription and adjusts your dose with you. If the dose is making you sick, the next step is usually a slower climb, and you can ask him by message instead of waiting a month.
How do I get started?
- Book a free 15-minute consult with Dr. Saremi.

- You can start your medication as soon as Dr. Saremi has reviewed your labs and health history.

Can I do this by video if I don’t live near La Jolla?
Yes. Video visits in CA, AZ, FL, GA, IL and TX on both plans. Premium members can also see him in person in La Jolla or Newport Beach. Testosterone and other controlled medicines are prescribed for California patients only.
Where is the La Jolla office, and when is it open?
Every visit is by appointment. Seeing Dr. Saremi in person is part of the Premium plan; on Essential, visits are by video.

More questions
Do you take insurance or Medicare?
No. The membership is self-pay, and HSA and FSA cards are accepted. If your plan covers your medication without a prior authorization, you can use it at the pharmacy; if it doesn’t, here is what to do when insurance won’t cover your GLP-1.
Is the medication included in the membership?
No. The pharmacy fills and bills the prescription. Labs are included in both plans.
What labs do I need before starting a GLP-1?
Dr. Saremi orders them at your first visit. For what they usually cover and why, see which labs to get before starting a GLP-1.
