Insurance Won't Cover Your GLP-1? What Wegovy and Zepbound Cost Without It, and What to Do
If your insurance won’t cover Wegovy or Zepbound, a doctor can still prescribe it, and you can buy it from the maker at a self-pay price. As of October 20, 2026, the Wegovy pen is $349 a month at every standard dose, and Zepbound starts at $299 a month. A denial is about who pays for the medicine. It says nothing about whether the medicine is right for you.
Why so many plans say no
Most employer plans still don’t cover GLP-1 medicines for weight loss. In KFF’s 2025 survey of employers, 16% of firms with 200 to 999 workers covered them, 30% of firms with 1,000 to 4,999 workers, and 43% of firms with 5,000 or more. About a third of the firms that do cover them require something first, such as meeting with a dietitian or joining a lifestyle program.
So ask your plan one question: is the medicine excluded, or covered with conditions? If there are conditions, find out what they are and whether your doctor needs to send paperwork. If it’s excluded, paying the maker directly is how you get it.

Self-pay prices from the makers’ own pages, checked October 20, 2026. They change, so confirm before you fill.

| Zepbound KwikPen (tirzepatide) | $299 at 2.5 mg, $399 at 5 mg, and $449 from 7.5 mg to 15 mg if you refill within 45 days. Refill later and those doses cost $499 to $699 |
The pills cost less: the Wegovy pill and Foundayo both start at $149 a month. See Wegovy pill vs. injection for those prices and how the pills compare.
Why not a cheaper compounded version?
Compounded and online versions aren’t checked by the FDA for quality, and the FDA warns that counterfeits can have too little drug or none at all. Dr. Saremi prescribes brand-name, FDA-approved GLP-1s only. With a brand-name pen you know exactly what’s in it and exactly what dose you took. More in Is compounded semaglutide or tirzepatide safe?

Common questions
Can a doctor prescribe Wegovy or Zepbound if my insurance denied it?
Yes. A denial means your plan won’t pay. It doesn’t stop a doctor from prescribing the medicine or a pharmacy from filling it at the self-pay price.
Is Zepbound cheaper than Wegovy without insurance?
At the starting dose, yes: $299 against $349. At 5 mg Zepbound is $399, and from 7.5 mg up it’s $449 if you refill on time, more than the Wegovy pen’s $349. Which one is right for you is a separate question; see switching between Wegovy and Zepbound.
Will these prices change?
They can. The makers set and change them, the Wegovy new-patient price ends December 31, 2026, and the lower Zepbound price depends on refilling within 45 days.
