Prepared by the team at Adonis Saremi, MDMedical review pendingPrices last verified October 28, 2026

Without insurance, you buy Zepbound at Lilly’s self-pay price. As last verified on October 28, 2026, a 28-day fill costs $299 to $449, depending on the dose and how you buy it. With Dr. Saremi, care on the Essential plan is $249 per calendar month, and the medication is not included, so medication plus care is a separate sum you can work out below. Labs are included in both plans. Scans, supplies and shipping aren’t included in these figures.

Prices are the makers’ own self-pay offers, read from their official pages. A price shows here only while we have checked it within the last 14 days and its offer hasn’t ended. Medication is bought from the maker or pharmacy, not from the practice.

How the numbers are worked out

  • The medication price is the maker’s published self-pay price for that dose, with the supply it covers.
  • The monthly figure is an average for budgeting: price × 365 ÷ supply days ÷ 12. A price the maker sets per calendar month is used as it is.
  • An introductory price is shown on its own and never stretched into an ongoing monthly budget.
  • Care with Dr. Saremi on the Essential plan is added as a separate line. Labs are included in both plans. The subtotal leaves out scans, supplies, shipping and anything we couldn’t confirm.
  • A price shows only while we’ve checked it on the maker’s own page within the last 14 days and its offer hasn’t ended.
  • What you choose and type in the calculator stays in your browser. No email, no account, nothing saved.

Common questions

Does the price change with the dose?

It can. Lilly's self-pay price depends on the dose and, for some doses, on refilling on time. The table above shows each dose we could confirm, with its condition.

Is there a different price for the vial and the pen?

They're sold as separate products, so the table lists each form on its own, with the prices and terms we could confirm for it.

Is the medication included in the membership?

No. Membership covers your visits and messaging with Dr. Saremi, and labs are included in both plans. The medication is bought separately, from the maker's self-pay program or a pharmacy, and scans are billed separately when you need them.

Does this check my insurance or whether I qualify for a discount?

No. It shows the makers' published self-pay prices and their conditions, and it can't tell whether you meet them. It doesn't look at insurance at all.

Can I pay with an HSA or FSA card?

HSA/FSA cards accepted. Whether a given cost counts as an eligible expense depends on IRS rules and your plan, so check with your plan administrator; we don't decide that, and this page doesn't promise it.
